CONTACT LENS AGREEMENT
The contact lens examination fee varies depending on your prescription and the type of contacts you are prescribed. If your insurance covers a routine eye exam that does include a contact lens fitting, a separate fee will be charged. If you have questions regarding specific fees, please contact us prior to your appointment date.
If an ocular condition prevents the doctor from accurately determining a prescription (i.e. red eye, corneal eye diseases, severe dry eyes etc.), you will not be given a prescription for glasses or contacts. Instead, you will be charged an office visit fee for that day’s visit (fee will vary depending on the condition).
If the doctor requires you to return for a contact lens follow-up, there is no charge for all CONTACT LENS RELATED follow-up visits for up to 2 months. If it takes over 2 months to finalize your contact lens prescription because you did not return for follow-ups or because your ocular health prevents the prescription from being finalized prior to 2 months, you will be charged $50 for each follow-up visit. It is your responsibility to come in prior to 60 days. We will not be contacting you to inform you that the 60 days is up. If it’s been over 4 months, you’ll be charged the original contact lens examination fee.
To further clarify: the all inclusive fee for 60 days of contact lens follow-ups is only for HEALTHY eyes with no ocular problems. If certain ocular conditions such as ocular allergies or dry eyes need to be treated in addition to the contact lens follow-up visit, there is a separate office visit charge for this.
A contact lens exam is required each year in order to receive a contact lens prescription. We will not extend your contact lens Rx past one year.
By signing the contact lens agreement, you are acknowledging that it is your responsibility to ask before your appointment date if you have questions regarding anything related to the contact lens fitting.
